Listed below is the admission criteria for the Mental Health Outpatient Facilities.

Completion of Intake Call (service recipient demographic sheet )

Insurance Card

Driver’s License

Social Security Card

Birth Certificate (For Service Recipients under 18 years of age)

Custody Paperwork for all C&Y service recipients placed out of parent’s custody.
